Good question.

If you're upgrading a recent crew over a recent crew, it's ok to just
do an upgrade.
If you're going from crew to stable or stable to crew, you should
uninstall and reboot, install again.

When you go from crew back to stable, you should delete
%APPDATA%/TortoiseHg.  This will be fixed in 0.7.6.
