I just saw it the one time, not at every session start.

signon-ui-service is installed, and I can DBus activate
com.canonical.indicators.webcredentials.

Maybe you can let me know what to do if it happens again?

  I was just doing Netflix and chill, when signon-ui created the
  attached dialog apparently out of the blue.

  What is it asking me to give credentials for, and why?
