Squeezebox Server _never_ modifies anything in your library, so don't
worry about that.

A 'clear & rescan everything' type of scan wipes out everything in the
SBS database and starts over from scratch. During this type of scan the
music library cannot be played (although it will be gradually built and
partially usable). A 'new & changed' scan picks up any newly added
files and changes to existing files, and the music library remains
usable during the scan. Typically, you only need to run a full 'clear &
rescan' occasionally. SBS sometimes requires it to correctly recognize
certain types of library changes. A 'new & changed' scan is much
faster. Use it when you make changes or add new albums.

Missing artwork is a common problem. Hard to say what could be causing
it in your case without knowing how you store covers. Historically, a
'clear & rescan' scan often had to be used to fix artwork problems in
Squeezebox Server, such as seeing the wrong covers for some albums.
Recent versions of the server have improved this and it's needed less
often.

If a full clear & rescan fails to index all of the albums in your
library, go to Settings > Information and find the link to the scanner
log.  Examining the log may give you an idea of where and why the scan
is failing.
