Have you tried the DVD-Audio Decoder and Watermark Detector plugin for Foobar 2000? http://sourceforge.net/projects/dvdadecoder/ I have had success with this on a variety of DVD-A's You can convert to flac (rip) from within foobar. Foobar 2000 is available from http://www.foobar2000.org/
