Pardon, of course it's simpler now (from the Trac docs):

#!/bin/sh

while read oldrev newrev refname; do
    git rev-list --reverse $newrev ^$oldrev  | \
    while read rev; do
        trac-admin /path/to/trac/env changeset added a_repository $rev
    done
done

This works fine for me so far.
