I had the same problem trying to generate an ISO to test with, so I really
do think the DVD is bad. (: I'll still see if there's anything I can add
to transcode to work around this, but I think the solution is going to be
"don't use that DVD recorder".
FWIW, I couldn't get mplayer to recognize multiple chapters in the DVD,
and it also gave me the warnings:

for vmgi_mat->zero_3 = 0x00000000010000000000000000000000000000
*** Zero check failed in ifo_read.c:977
for tt_srpt->zero_1 = 0x3650
which I've never seen for any other DVD.
