:-)  I'm used to deploy applications and sometimes my clients don't hire a
support or maintenance contract, so I have to optimize things for their data
volume.  If it gets too slow too early, then it is bad for me.  Of course, I
can't predict everything but I try following some patterns that I've learnt
with time.

If I could choose, I'd go for something like you do: optimize when it is
needed, monitor queries, etc.  It is hard doing that for several clients,
though... :-)
