Than it means something has changed in TYPO3. If the same RealURL works differently with different TYPO3 versions, than it is obviously TYPO3 :( The bad site of it is that locating the problem will be much harder. Can you report the bug to the bug tracker under "tx_realurl" with "major" severity? Please, mention that I asked you to report it.
