I reported this issue (or at least a related one) last week. I was told
it will be fixed shortly, but the author is on holiday right now.

One of the major issues I had (and still have) with the extension is
that it doesn't work correctly if you don't use a "username" field in
your registration form (my intention was to have the email be the
username). Additionally, the username can not be an email address right
now.

  Same thing here, but no luck.
Devlog o typo3 logs tell nothing.
I'll check on Apache logs to see if at least there I can find anything.

  I've only set the following values in my registration form:

Default User Folder: My sysfolder that contains feuser records
Default User Groups: The user group for new accounts
Approval Check: Admin approval
[X] Send User E-Mail?
[X] Send Admin E-Mail?
Sender Name: My Company
Sender E-Mail Address: My eMail address
Admin Name: My Name
Admin E-Mail Address: My eMail address

No TypoScript or anything out of the ordinary.
I also haven't set a Redirect Site ID for Registration.

Maybe the Log shows some interesting errors?

  Hi Nicola,

I was experiencing the same issue when I recently started using
datamints_feuser. If I'm not mistaken, it was caused by
missing/invalid
eMail parameters (like "Sender Name" and/or "Sender E-Mail Address").
