you could start vom scratch (or you have your own configuration to start with)
or you could use a preconfigured distribution someone else has uploaded to TER
(in such a distribution there are preinstalled some extensions and there are 
many lines of TypoScript the author thinks they are usefull)

but this depends on YOUR needs... i normaly use one of this three ways:
1.) from scratch and install my own basic distribution
2.) themes distribution
3.) t3sbootstrap distribution

for learning the offical distribution package is a good way, but i imho 
wouldn't use it in productional usecases...
