Activity into the DBMS can come from any product, whether it is .NET aware or not (ASP, VB6, Java/Bridge, PHP/COM, etc). mv.NET now has bi-directional updates so that the DBMS can also trigger activity in remote environments, which may be more in line with what you want. As soon as people realize UO doesn't scale they often start considering very expensive solutions like RedBack.
