Bom, é estranho, pq parece estar tudo certo.
De qualquer forma, eu sugiro que vc tente substituir as sessões monitor
e screen do seu arquivo por essas:

Section "Monitor"
    Identifier "T730SH"
    Option "DPMS"
    HorizSync 30-71
    VertRefresh 50-160
EndSection

Section "Screen"
    Identifier "Default Screen"
Device "NVIDIA Corporation NV34 [GeForce FX 5500]"
    Monitor "T730SH"
    DefaultDepth 24
    SubSection "Display"
        Depth 24
        Modes "1600x1200" "1280x1024" "1024x768" "800x600"
"640x480"
    EndSubSection
EndSection

Essas configurações eu montei com base na sua placa e monitor, e no
xorg.conf q o driver da nvidia gera pra mim através do nvidia-settings
