If all that doesnt work and the latest packages dont help either maybe you need a dummy password too or something. You could also try the latest network-manager with the broadband wizard - for that install libmbca0 and upgrade to latest. after removing your broadband connection from nm-connection-editor and replugging in your device you should get a wizard that allows you to select your exact provider info.
