What version of NM are you running? Maybe it works better when you try
multiple times?

If all that doesnt work and the latest packages dont help either maybe
you need a dummy password too or something. See
https://wiki.ubuntu.com/NetworkManager/Hardware/3G for providers that
are known to require that.

You could also try the latest network-manager with the broadband wizard
- for that install libmbca0 and upgrade to latest. after removing your
broadband connection from nm-connection-editor and replugging in your
device you should get a wizard that allows you to select your exact
provider info.

Also for GSM you should use *99# as number and 777# for cdma.
