I think he means he's trying to launch an application he added to the favorites menu. At any rate, I got the same thing when trying to launch Firefox from the favorites pane; it was placed there a while back, before the "add to favorites" click option picked up the application's icon.
Put a different way, at some point in the past, I added Firefox to my favorites menu and it only showed up as a gear, and today when I tried to launch it I got this error.
