I've been monitoring, and as as near as I can ascertain its a
combination of flash adverts and tabs.

When there is flash content and I deliberately give it focus, ctrl-W
stops working. While thats annoying as heck, its not the primary issue
here. 

So what I think is happening is that when working with many tabs,
somehow a flash advert gets focus on a tab, and ctrl-W then stops
working until you click on the page outside the advert. 

I am *totally* sure I've not been tabbing or clicking into the adverts
though.
