Yup, I can pretty much just echo what the others are saying. This seems to happen ONLY with Xinerama enabled -- when I don't enable Xinerama it doesn't occur. First I thought this had something to do with mouse grabbing since it only happened (for me) in VirtualBox and KVM, but now it seems that the trigger (at least for my system) is simply high CPU load.
I've tested with KDE4.1.2 and xmonad, both display exactly the same symptoms -- everything runs normally, but X does not receive mouse events (as reported by xev).
