I am not certain exactly how to reproduce it, however I think it has to do with some combination of switching between fullscreen and normal mode and also toggling the display of the controls (I specifically use they shortcut ctrl+h). I am able to do both and have them function as expected right now. However I did just experience this again about an hour ago (my second time). If I am able to pinpoint exactly what I am doing when this happens I will be certain to post it.
