Here's some extra info on my problem (which may not be the same as
Derek's).

Restarting X with Ctrl-Alt-Backspace brought sound back.

I tried suspend/resume 3 or 4 times after this and sound was fine.

Then, I watched a Flash video (sound was working). With the Flash applet
still open, I suspended and resumed, and sound was gone again.

In the PulseAudio Volume Control I terminated the stream "ALSA plugin
[firefox]". Now non-Flash sounds worked, but Flash videos would freeze.

Restarting Firefox brought back Flash videos.

This pattern is consistent: suspend/resume with a Flash video open
breaks sound output until I terminate the stream/restart Firefox. The
behavior is similar to a previous problem I had (although, in this case,
the PulseAudio server does not seem to be dying):
