It happened for me again, after updating to Firefox 3.0.4. I removed and re-installed Firefox 3.0.4 which didn't help... or so I thought. I day later it was working again. I don't know if I rebooted or not, in the meantime.
BTW, to answer the earlier question, yes, the icon appears.
