Please also collect some info from your monitor:

  xrandr --query
  ddcprobe
  get-edid | parse-edid   # install read-edid package

As well, we may need to have the PCI ID of your video card.  Run `lspci
-vvnn > lspci-vvnn.txt` and attach that file.

Also, if we need to forward this issue upstream, upstream will wish for
you to test against a newer version of the xserver.  ISOs of our alpha
releases are available at cdimage.ubuntu.com.  It would be great if you
could boot the alpha-2 livecd at some point and verify the issue still
exists there (if it doesn't, I may be able to ferret out a quirk to fix
your monitor once we have the above data).
