Unplug the device and rmmod/modprobe the appropriate modules until the cycle should work again, and run lshal > lshal_1.txt while the device not the monitor) is unplugged. Now open a separate terminal and run lshal -m leave this running... Then do your device plug in and unplug cycles until it doesn't work anymore and run in the original (or new) terminal, run lshal > lshal_2.txt while the device (not the monitor) is unplugged. Please upload the lshal_1 and _2 files and copy and paste the lshal -m output here as well.
