After further investigation, my problem is definitely related to this bug. I was mistaken that xmodmap works with the USB keyboard initially, it does not work. Xorg.0.log is indicating that every few minutes, the USB keyboard is reloaded, because of a Read Error. That is why I lose my xmodmap settings every few minutes.
