I encounter this bug regularly. I use CoolPreviews, DownThemAll!, Flashblock, Foxmarks Bookmark Synchronizer, PDF Download and Ubuntu Firefox Modifications 0.7b1. However, I think that none of these is responsible or in connection with the bug.
