I have the exact same problem. My printer, a Brother MFC-8440, will print the header and footer when using the Windows driver and Firefox (1.5 or 2.0) but when using the Linux equivalent, no header or footer is to be seen. I don't know I fix, but I do know that this is the root of the problem.
