Thanks for taking the time to report this issue and help improve Ubuntu. For the GeForce FX 5200 you should use the 173 driver, not the 96 driver. Please install the version 173 driver, reboot, and then attach /var/log/Xorg.0.log and /etc/X11/xorg.conf.
