Public bug reported:

When grub says

  Error 15 file not found

it would IMMENSELY helpful if it included the name of the file it could
not find.

Was it the kernel it couldn't find?
was it the initrd image?
was it something else?

a person has to be psychic to figure it out!?!

Please please please include the name of the file in the error message.

How to reproduce: take a file system image with tar (carefully) but don't fix
menu.lst
boot using a rescue disk, mkfs the partition, mount it and restore the tarball
into it.
Run grub-install (which reports success).
Try to boot... mwahahahahaha
WFT? Guess what the problem is. Nope, wrong, guess what the problem is.
