I haven't installed the LBM modules, but the channel 12/13 is also missing 
after startup with the 
"default"? ath5k kernel module

To fix this, i did:
ifdown wlan0
modprobe -r ath5k
modprobe ath5k
(if up wlan0)

After that, all 14 channels are active.

I think, it is not a good solution, but it works (e.g. /etc/rc.local)

Anyone knows the correct solution for this bug?
