It seems to me that gscan2pdf is trying to read the image before it has been fully written.
scanimage was modified in 1.0.20 to buffer the image, and to write it only when complete. Please try the attached patch, which flushes the filesystem buffers immediately before importing the image. If this doesn't work, the only other idea I have is to hack the import code to monitor the image as it is written and only import it when the file size matches the PNM header.
