We need you to test against Karmic kernel (cx18 used by you is 1.0.0 and doesn't include several enhancements in 1.2.0 that may solve your problem) if this doesn't work we'll need you to test against latest mainline PPA kernel. Your card requires high IO from your PCI bus and it seems to get choked at the boot time by some other device or because a shared interrupt. That avoids it to detect properly the firmware so it won't work.
