Two things: 1. try the latest daily builds from: https://edge.launchpad.net /~network-manager/+archive/trunk
2. if that does not work, create a file /etc/defaults/NetworkManager and put the following line in it: export NM_PPP_DEBUG=1 and restart everything. Attach the modem-manager --debug output and the syslog text for the startup and the first connect attempt to this bug.
