I am experiencing too this strange bug after upgrading to 9.10: some icons are missing using human icon collection.
1) all icons in system menu 2) some icons in places menu (connect to server, search for files, recent documents) 3) icons of firefox addon "noscript" menu (this is probably a different issue...) Unfortunately the same happens with every collection of icons (gnome, tangerine, oxygen, ...)
