I can confirm this problem exists on Karmic using Openoffice.

"Failure loading aff file /usr/share/myspell/dicts/en_ZA.aff" is written to the 
console after launching oowrite from the console.
Simple words such as 

I made the error go away (note, I don't consider myself an expert in the
affix file format - and thus don't know which south african grammar +
spelling rules will be violated ), by copying the en_GB.aff file over
the en_ZA.aff file

sudo cp /usr/share/myspell/dicts/en_GB.aff
/usr/share/myspell/dicts/en_ZA.aff

it seems that there's a parse error on the existing en_ZA.aff file (and
indeed may have something to do with OP's duplicate REP table errors).
