Getting the same fading effect with a fresh install of Karmic on a Dell Dimension E520.
lspci lists the sound hardware as 00:1b.0 Audio device: Intel Corporation 82801H (ICH8 Family) HD Audio Controller (rev 02) Steps to reproduce: 1. Plug headphones into the front panel. 2. Open Places->Home Folder->Examples->Ubuntu Free Culture Showcase. 3. Double-click InTheCircle.oga. Playback starts normally, but sound fades smoothly and is totally inaudible by 0:44. Closing the movie player before sound has totally faded, waiting ten seconds, then double-clicking InTheCircle.oga again will start playback at a much reduced output level, as if the fade has been going on in the background even with no sound playing. Leaving the movie player closed for a couple of minutes is enough to reset volume to normal.
