I tracked the problem down to a single line ~/.kde/share/config/konquerorrc: 'CurrentEngine=locate' in the [SearchBar] section. When running from the command line, Konqueror would print 'findServiceByDesktopPath: searchproviders/locate.desktop not found' to STDERR. I removed the file and let Konqueror rebuild it, and search bar has appeared in the toolbar again.
