Same issue here with Kubuntu karmic. I first noticed it with amarok and skype and then with flash. all of them were set to default. using intel hda on dell latitude e6400. I never could play sound by two applications simultaneously.
Pulseaudio is not installed on my system, nor its configuration-files are present.
