I having similiar conditions too. My sound card is gone from pulseaudio preferences. System -> Prefrences -> Sound -> hardware.
This is happens after I try the hibernate. But it can fixed by typing command $sudo alsa force-reload Now I put that command on /etc/rc.local. I know that's a little tricky.
