Installing libxine-extracodecs should have solved the problem, but I'm only reading the documentation from the wiki and forums (as I do not use Kaffeine or Xine).
>From looking at the package it installs "xineplug_decode_mad.so" in the usr/lib/xine/plugins/1.1.1/ directory, but you seem to be using 1.1.2. Just to check, are you running Ubuntu / Kubuntu 6.06 and are you installing Xine / Kaffeine from the official repositories?
