Would you please add the information requested from the "Reporting Sound Bugs" section of https://wiki.ubuntu.com/DebuggingSoundProblems as separate attachments? If you are running Jaunty or Karmic this can be done automatically if you run "apport-collect -p alsa-base 504020" (without the quotes) from the terminal.
