Thank you for taking the time to report this bug and helping to make Ubuntu 
better. Unfortunately we can't fix it without more information.  Would you 
please add the information requested from the "Reporting Sound Bugs" section of 
https://wiki.ubuntu.com/DebuggingSoundProblems as separate attachments? If you 
are running Jaunty or Karmic this can be done automatically if you run 
"apport-collect -p alsa-base 504020" (without the quotes) from the terminal.
If you are running Karmic, please read through the following wiki page as it 
resolves a lot of common audio problems.
