Ok, using ignore_dB=1 restored the old behavior. And the cause turned
out to be more or less what I expected. Pulseaudio is merging the
hardware mixers into one big mixer. The 0% mark on my Master mixer also
says "-48.00 dB", and the 0% mark on my PCM mixer gives me "-51.00 dB",
which means that the maxers together have a total of 100 dB or dynamic
range. However, I don't think the actual hardware (the speakers) can
produce this dynamic range, so the lower end of the volume slider is
just completely silent.

So maybe the *real* problem is that I need a way to specify the max and
min volumes in dB, and let Pulseaudio figure out the rest. I certainly
don't need 100 dB of dynamic range.
