Public bug reported:

Binary package hint: evince

When a PDF with Forms is opened in Evince, radio buttons are not handled
correctly. You can click a spot and a tick mark will appear. Click an
opposing radio button and you get ANOTHER tick mark -- the first tick
won't disappear as expected. Furthermore, when Evince starts, I get
this line once for each instance of a radio button in the document:

Error: FormWidgetButton:: unable to find the on state for the button

This makes filling forms extremely difficult.
