This doesn't appear to be a graphics driver bug. The symptoms suggest that you're not running a window manager. The default window manager for cards with 3D is compiz, which you seem to have removed at one point, and for other cards is Metacity. Without a window manager running windows stay where they're created - in the top-left, you can't move them or change their stacking order, and they don't have titlebars.
