Thank you for the updated dump. It seems that intel_gpu_dump output and i915_error_state is still incompatible, but at least now we know that this is not due to the limited debugging capability of the ubuntu kernel. Maybe this tells the developers something useful.

Please register at bugs.freedesktop.org (if you haven't already) and add yourself to the CC field in the upstream bug report. You will then be able to work directly with upstream to get this bug resolved. If you have any ubuntu-specific questions (e.g. how do I test what they ask me to test?), you can ask here at the downstream report so that the upstream developers are not bothered with that. It is possible that they will ask for the dri_debug info with an updated userspace.
