As of last December, with the version of ~/.config/monitors.xml attached to comment #3 present, the problem occurred. With it absent, it did not.
Several updates to the kernel and other components have happened in the intervening time. gnome-control-center is currently at version 1:2.28.1-0ubuntu1, and according to Update Manager all installed packages are up to date. I currently have no monitors.xml file, and don't know exactly how it came into existence. I just tried restoring the bad monitors.xml file, and the observed behavior is that I was able to log in, got a complete desktop at the expected display resolution, but had no mouse cursor at all. Driving by dead reckoning, I was able to poke at a few things to demonstrate that X and the desktop seemed to be otherwise functional, and succeeded at logging out again.
