We've tried a number of things now, including the workarounds above and updating the firmware on the system and RAID controller. More details:
This seems to be a GRUB issue, actually. This occurs immediately upon attempting to boot, and we can't invoke the grub menu at all. At this point the firmware change has rendered the system unable to install correctly; this may or may not be a related problem.
