Same here.

We got this one on a machine after a fresh update from lucid-proposed.
None of the usual workaround (reconfiguring, reinstalling udev) fixed
the situation. Only a complete rebuild of the initramfs images did the
trick.

- chroot in your broken install from a liveUSB/CD 
- sudo update-initramfs -u -k
- reboot

All sorted. It was a pain to narrow it done as it first seemed to be an
LVM or disk issue, but no.
