I've found out more information about this problem. Most applications,
such as gedit, gnome-terminal and even thunderbird, have a separate
application title (which we don't see used in Gnome 2) and the window
title. This means that the application title never changes (which is why
gnome-shell doesn't poll for it to update), and is stored in the super-
title-bar, and the window title, which may change, is in the usual
location.

Firefox is the only application that disrespects this and has the window
title as the application title too. The application title should be
"Firefox", not "<page name> - Firefox" -- the latter should be for the
window title only.
