I've found out more information about this problem. Most applications, such as gedit, gnome-terminal and even thunderbird, have a separate application title (which we don't see used in Gnome 2) and the window title. This means that the application title never changes (which is why gnome-shell doesn't poll for it to update), and is stored in the super- title-bar, and the window title, which may change, is in the usual location.
Firefox is the only application that disrespects this and has the window title as the application title too.
