Three additional points. 1) The title bar is correctly updated when Epiphany stops refreshing. 2) This persists even if I disable the global menu. In this case menus are not drawn either, although initially Compiz creates windows for them (the outlines are visible) 3) Most importantly, this worked on Monday (Oct 4), so it must have been caused by a recent update.
