Now I think I know why there are overruns and underruns: there is something wrong with the latencies.
The attached patch disables the special handling of the playthrough case and uses the user-settable latency for both capture and playback. The patch also contains a few diagnostic output statements that print out the suggested latencies (ignored) and the latency set in the preferences, which are the ones used.
