Due to some self inflicted system injuries (I blame pulseaudio) I hit
his error.  The reason for this is the gnome-settings-daemon is not
working so GTK does not pick up the correct icon theme so it defaults
back to hicolor, which does not have icons for "window" or "gtk-missing-
icon".  These icons exist in the gnome-icon-theme package and in GConf
this is the default, and is also in Clearlooks, which is the default for
the overall theme.
