I've found a hack to get this working. The file:
 /var/lib/alsa/asound.state

has two important fields set to false:

                name 'HeadsetL Mixer Voice'
                name 'HeadsetR Mixer AudioR1'

when I manually edited the file and then ran "alsactl restore" I was
able to get sound working.

I'm not sure why the default file is generated incorrectly and why I
couldn't enable this using the alsamixer GUI
